  1. Check all that apply. Only one team manager, scorekeeper and equipment manager will be required per team.

  1. FAITH BOND
    A $100 Faith Bond cheque dated September 1, 2018 payable to Dorchester Minor Baseball Association will be required per player to be collected at the uniform fitting night. The cheque will only be cashed should the family not fulfill one of the requirements below. Please note it is the responsibility of the parents or player to provide volunteer time for the Dorchester Minor Baseball Association.
    • Activities to work off the bond are: Sponsorship ($250 or $500 levels), committee member, house league convener, coach, assistant coach, score keeper (1/team), equipment manager (1/team), team manager (1/team), Rookie Development parent involvement (4 nights/year)
    • Other volunteer options are tournament volunteers, league playoff tournament volunteers and provincial tournament volunteers.

    REFUND POLICY
    • Requested prior to April 21, 2018 – 100%, After the first game – 50%. No requests for refund will be accepted after June 1, 2018.
    • Raffle Tickets are non-refundable.
    • All refunds are subject to an Administration Fee of $30.
    • The refund form and instructions can be found at www.dorchesterbaseball.com.

    LATE FEES
    Late registration fee of $50.00 per player is in effect starting March 31, 2018, and depends on space availability on appropriate team.
    • Late fee does not apply to Rookie Development.

    EQUIPMENT REQUIREMENTS
    • Jocks, gloves and batting helmets are required and are to be provided by the player for all divisions.
Payment
A $50 late fee will be added to the registration fee after March 30, 2018.
  1. In lieu of the $100 faith bond cheque and providing volunteer time you can purchase a second book of raffle tickets. The tickets will be provided to each player selecting this option at the uniform sizing event.
  2. A fundraising fee of $50 is required per registered player. A book of raffle tickets will be provided to each player at the uniform sizing event.
  3. Once you have completed and submitted this form, you will be directed to Paypal - a secure, online payment processing website which accepts major credit cards and Paypal accounts. Your registration IS NOT complete until you have submitted payment. Choose guest payment to pay with credit card instead of Paypal account.
Tax Receipt
Dorchester Minor Baseball programs qualify for the Child Fitness Tax Credit. The emailed receipt of your registration form can be used for tax filing purposes.
