  1. This form must be completed to request a refund for the 2021 season.  Upon submission of the request, please allow 2-4 weeks for processing.

    All tryout fees paid for the 2021 season will be refunded in full (automatically).  

    All raffle ticket fees paid for the 2021 season will be refunded in full (automatically).

    Registration fees will be refunded at 100% if the request is submitted before June 30, 2021.  
    Registration fees will be refunded at 50% if the request is submitted before July 31, 2021.

    A $25 administration fee is applicable to all refund requests after June 15, 2021.

    There will be no refunds after July 31, 2021.

    All refunds will be issued by cheque and mailed to the original payee.

    No refund will be issued until any equipment, uniforms or property of Dorchester Minor Baseball Association has been returned in full and intact.

    LATE FEES ($50) are NON-REFUNDABLE
